OFFICIAL RULES FOR ANSELL MICRO-TOUCH CUSTOMER FEEDBACK PROMOTION

NO PURCHASE NECESSARY. The Ansell Micro-Touch Customer Feedback Promotion (“Promotion”) begins February 15, 2012 and ends on March 15, 2012. The Promotion is sponsored by Ansell Healthcare Products LLC, 200 Schulz Drive, Red Bank, NJ 07701 (“Sponsor”). Void where prohibited. Eligibility: The Promotion is open to natural persons 18 years of age or older who have reached the age of majority in their state of residence, who are legal residents of the 50 United States or the District of Columbia who are healthcare professionals. Sponsor’s employees, officers, and directors, and the employees, officers, and directors of Sponsor’s parent companies, affiliates, subsidiaries, advertising and promotion agencies, and any entity engaged in the production of the Promotion and the immediate families of each, are ineligible. All federal, state, and local laws apply.

To Enter: Participants must use the provided Micro-Touch powder-free colloidal oatmeal nitrile exam product and fill out an evaluation form (both items to be provided by Sponsor), and fax the form to H. Campbell at 732-345-1575. After completing and returning the evaluation form, participants will be entered into the Promotion. Limit one entry per person. Forms must be fully and accurately completed to be valid. All entries must be received by 11:59 PM ET on March 15, 2012. Entries submitted by any other means or after the entry deadline may not be accepted. Sponsor is not responsible for any problems of any kind with the completion or submission of entry forms, whether mechanical, electronic, or human. Entries that are incomplete, defective, altered, illegible, mutilated, forged or irregular in any way, or not in full compliance with the Official Rules, are void.

Winner Selection and Notification: One (1) winner (“Winner”) will be selected by a random drawing by a representative of Sponsor to occur on or about April 1, 2012. Odds of winning depend on the number of eligible entries received. Winner will be notified by telephone or electronic mail within ten (10) days of drawing date.

Prize: Winner will receive one (1) Amazon Kindle Touch (ARV: $99). No substitution or transfer of prize, except at sole discretion of Sponsor. Any applicable taxes are sole responsibility of Winner. All prizes will be awarded.

General Terms: Winner may be required to execute an affidavit or declaration of eligibility, waiver of liability, and publicity release, which must be returned within fifteen (15) days of notification. Failure to comply with any such requirements may result in disqualification, and an alternative winner may be randomly selected from remaining eligible participants. By entering the Promotion, participants consent to use of their name, image, likeness, and comments from the evaluation (subject to editing) for Sponsor’s advertising, promotional, and publicity purposes without additional compensation, except where prohibited by law. All entries become sole property of Sponsor and will not be returned. By entering, participants agree to be bound by these Official Rules, and release Sponsor, its agents, parent companies, subsidiaries, and affiliates, and each of their respective officers, directors, employees, and agents from any and all liability, loss, or damage arising out of their participation in the Promotion or with respect to the awarding, receipt, possession, use, and/or misuse of any prize.

Sponsor is not responsible for late, lost, misdirected, incomplete, incorrect, damaged, misdirected entries or prize claims. Sponsor is also not responsible for incomplete, interrupted, or scrambled transmissions, unavailable web sites, networks, servers, or systems, or other connection or transmission problems, technical malfunctions of a computer, fax machines, software, web site, or any combination thereof, problems associated with any virus or other damage caused to any participant’s system or other problems of any kind, whether mechanical, electronic, or human. Sponsor reserves the right to suspend and/or cancel the Promotion or change any element hereof, with or without notice, at any time, due to printing, fax, computer or network errors, or other mistakes in the production or operation of the Promotion.

Personal information collected as part of this Promotion may be shared with Sponsor’s promotional partners to the extent necessary to fulfill prizes. Except as otherwise contemplated in these rules or in any publicity release, information collected by Sponsor in connection with this Promotion will be used by Sponsor in accordance with Sponsor’s online privacy policy posted at http://www.ansellhealthcare.com/temps/privacy.cfm.
